FAA Medical Certification

We also offer Airman Medical certification physical examinations, including First Class examinations. Dr. Thrasher is a Senior Aviation Medical Examiner. Prior to examination, each airman needs to complete and submit the FAA Form 8500-8 found on the FAA MedXpress website. Airmen Alert! Prior to FAA Physical Examination with AME:

  • Complete online FAA Form 8500-8 (FAA MedXpress)
  • Electronically submit 8500-8
  • Write down Confirmation Number showing successful submission. Bring Confirmation Number to appointment with AME.
  • Print a copy of 8500-8 and bring to appointment with AME .

 

DOT Medical Examinations

All our physicians are Certified Medical Examiners registered with the Federal Motor Carrier Safety Administration’s National Registry of Certified Medical Examiners.
